Data collection tool for Revamping PHC facilities project by KP Govt

To improve the health outcomes of people of KP living in rural areas by provision of primary healthcare services, with special focus on emergency services and services to women of reproductive age, newborns, and children by enhancing access to quality health services targeting the poor and vulnerable groups in rural areas of KPK, through upgradation of primary health care facilities i.e. Basic Health Units and Rural health Centers in phased manners. This app is a tool for collecting data from various end points regarding key performance indicators of the project.
