The new My Medical Record app has a fresh new look with improved user experience

My medical record is your personal health record (PHR) provided by University Hospital Southampton NHS Foundation Trust. Anyone who is (or has been) a patient at University Hospital Southampton can sign-up for an account (and then login) using their NHS Login.

My medical record stores and displays personal and medical information, some of which will be loaded from hospital systems, including blood results, clinic letters and outpatient appointment information.

My medical record helps users to get more information related to the management and treatment of various diseases or conditions, and also provides a functionality to message their healthcare providers and/or clinical teams about their condition.

My medical record allows users to manually enter and track their activities and exercises. It also allows you to add your own information about your health, including details which may be being monitored as part of your current treatment, such as your weight or blood pressure.
