This application is intended to be used in the follow-up of individuals who are primary care physicians and family health workers, especially those who have prediabetes and diabetes. Family health elements on nutrition, vaccination, follow-up, foot care, skin care in diabetes; it also contains information on healthy nutrition, physical activity, and cardiovascular risk assessment where necessary in all applicants.
