Data Collection Made Easier

United Nations Children’s Fund (UNICEF) in Chhattisgarh is working closely with the Department of Health and Family Welfare in areas related to system strengthening, capacity development, facility assessment for improving the impact of the health service delivery system.

This application has been developed to collect data from District Hospital, Community Health Centre and Primary Health Centre for the assessment of services provided, HR, physical infrastructure, equipment, drugs, furniture, quality control, fire and electrical tool at the delivery point. The assessment checklists are based on the Indian Public Health Standards (IPHS) norms and guidelines laid down for the health facilities.

This software is developed by SDRC (Siddha Development Research and Consultancy), a Bhubaneswar based company.
