Bluetooth Tethering Manager

Version 1.8:
- completely rewritten app core,
- LTE icon bug fixed,
- Lollipop+ problems while disconnecting from WiFi and reconnecting
- adapted to material design,
- added wizard page,
- added non-intrusive ads

New permissions:
- modify system settings - requied on M to enable bluetooth tethering
- approximate location - to obtain list of WiFi networks in neiughboorhood, fixes problem on Lollipop+,
- USB storage - keeping files on card, needed by an app
