The "Life Reactivation Mobile App" combines the participation of technology, medical care team and carers to establish a new home rehabilitation service model for the elderly. Through the use of this program, the elderly’s healthy life model will be reorganized, and rehabilitation training and health monitoring will be carried out at home.

The Social Service Department of the Christian Hong Kong Lutheran Church received donations from the Hong Kong Jockey Club Charities Trust Fund to implement the "Life Reactivation Project: The Jockey Club Home Rejuvenation Milestone" in 2018, and establish home rehabilitation for the elderly through the "Life Reactivation Mobile App" New model of service. This service combines the participation of technology, medical care team and carers to restore a healthy lifestyle and rehabilitation training for the elderly. Just install the "Life Reactive (Elderly Version) Tablet App", and the elderly can use the following services at home: Active health monitoring, self-help rehabilitation training, brain exercise program, life reactivation channel, life reactivation schedule and life online communication.

"Life Reactivation (Carers Version)" mobile app comes with the following functions:

1) "Health Monitoring": On the app, the carer can instantly know the health index of the elder's self-measured body temperature, weight, blood pressure, blood oxygen, blood sugar, etc., and keep abreast of the elder's latest health status at any time.

2) "Life Schedule": On the app, carers can keep track of the life schedule designed by the medical team for the elders, and encourage them to regularly measure various health indicators, exercise, play games, and watch videos on the app to make life easier More regular.

3) "Online communication": The carer conducts an online conversation with the elder on the app to facilitate communication.

4) "Latest News": Carers can watch useful care information on the app.

Service Target: The partner organizations of this project and the carers of the elderly who are receiving home services.
