New:
- Career highscore character can be selected in Arcade mode

Tweaks:
- Secret names in Career mode now get random equipment instead of a specific piece of equipment
- Punches now do a small amount of damage
- First 3 enemies in Career mode will never have shields
- Surgeon revival screen in Career mode doesn't load with thumbs up
- Surgeon revival max HP loss lowered slightly
